Photo Release

By agreeing, I hereby authorize The Toronto Local Charity Foundation to use my picture/video to promote the activities of the agency. Methods may include website, newsletters, annual report, posters, press releases etc. I waive any right to inspect or approve the finished text, images or any media – print or electronic, that may be used with them. These photographs, interviews, and/or video (digital/tape) may be used for promotional brochures, promotions, or showcase of programs on our websites, in social media, showcase of activities in local newspapers, and other not-for-profit purposes. This permission includes, without limitation, the right to publish such personal information and images in the The Local Charity intranet, reports, newsletters, and public relations/promotional materials, such as: The Local Charity website; The Local Charity social media accounts; advertisements; newspapers; magazines; television; posters; display boards; and slide presentations; or otherwise displayed to the public, or used for other educational, information or training purposes – either in whole or in part. These images and personal information or comments, may appear in any of the wide variety of formats and media now available to The Local Charity and that may be available in the future, including, but not limited to print, broadcast, videotape, CD-ROM, and electronic on-line media. 

I have had sufficient time to review and seek explanation of the provisions contained above, have carefully read and understand them, and agree to be bound by them. I voluntarily give my consent and agree to the waiver, release and indemnities contained herein.

In the event that I choose to withdraw my consent, I shall notify The Local Charity HR Department, in writing at info [at] thelocalcharity.com. My photograph/personal information shall cease being utilized for the above.
